RISKMAP, a specialist in risk management for the mining industry has recently launched the findings of a study entitled, ‘Why Safe Organisations Fail’ on their website.

This study into mine disasters, originally commissioned by the NSW Minerals Council, started in 1997, resulted in several reviews of subsequent disasters, also outside the mining industry and in North America. The findings in this study were reported at international conferences since then, including IAPA, Pacrim, AIMM and ASSE.
